Chronic Care Management
Beauregard Health System partnered with CareHarmony to provide a Medicare-sponsored service for eligible patients, focusing on Chronic Care Management and empowering patients to better manage their chronic conditions. The program is designed to give patients greater support and access to care between office visits, even while at home.
"Some patients may leave their doctor appointments with questions they forgot to ask, or may just need help refilling a prescription. This valuable service can also prevent small health concerns from turning into bigger ones," says Nicole Clanton, RN, BSN, CCM, Population Health Manager.
CareHarmony Care Coordinators are specially trained to tailor treatment to a patient's individual needs, including assistance with scheduling appointments, explaining how and when to take medications, coordinating home health or medical equipment needs, connecting patients with health education resources and much more. Additionally, patients enrolled in the program are granted access to a 24/7/365 Care Coordinator Line. Since the program began in June 2021, Beauregard Health System has been able to assist over 600 Medicare beneficiaries with their individual health needs utilizing Chronic Care Management services.
