IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

NOTICE: IRS Form 1095-C Availability

In compliance with the Affordable Care Act (ACA), BHS is required to provide Form 1095-C to certain employees and individuals. Form 1095-C includes information about the health insurance coverage offered to full-time employees by BHS for the previous year. Due to the recently enacted Paperwork Burden Reduction Act, employers are no longer required to automatically distribute Form 1095-C to all employees. Instead, these forms will be provided upon your request.

Form 1095-C is now available upon request.

Requests can be made at any time, and the form will be provided to you via mail within 30 days of your request.
