Family Health Clinic in DeRidder Recognized for Exemplary Performance on Nationally Recognized Clinical Outcome Measures
Edwin Bonilla, MD, Chris Granger, MD and Jonathon Sossamon, MD of the Family Health Clinic in DeRidder received a 2018-2019 GOLD TMF Physician Practice Quality Improvement Award for demonstrating excellence in health care quality through effective care management and the reliable delivery of preventive health services.
Presented by TMF Health Quality Institute and its partner medical associations, the award honors physician practices for improving patient outcomes while applying technologies and strategies to support effective care management and comprehensive preventative health services. The team from the Family Health Clinic are among more than 3,200 health care providers in seven states and the territory of Puerto Rico to receive the award.
“Providers like the Family Health Clinic are setting standards for providing care that is effective, reliable and patient-focused,” said Gary W. Floyd, MD, chair of the TMF Board of Trustees. “Their leadership in process improvement is helping to transform health care and create better outcomes for their patients.”
The TMF Physician Practice Quality Improvement Award is based on the four categories of the Merit-based Incentive Payment System: Quality Measures, Promoting Interoperability, Improvement Activities and Cost/Utilization. Practices were assessed on clinical outcome measures for the 2018 calendar year.
The award criteria aligns with national health priorities established by the Centers for Medicare & Medicaid Services to improve patient care and satisfaction, promote effective care management and increase the use of health care technology.
